We have hardcopy forms at our front desk. You can stop by and fill them out there.

1. You must fill out our child information form (per child) and email it to childcare@athensmcminnymca.org or drop it off to the front desk.

2. Register online and complete this step with payment of the registration fee. The waitlist option will not require a payment until you are moved to the active list.

3. Complete the bank draft agreement form and email it to childcare@athensmcminnymca.org or drop it off to the front desk.

4. Complete the suncreen/medication form and email it to childcare@athensmcminnymca.org or drop it off to the front desk.

5. Fill out the financial assistance form as needed and email it to childcare@athensmcminnymca.org or drop it and the household income documents off to the front desk.

6. The childcare team will reach out to you after completing these steps. If you do not complete these steps, you will not be considered as fully registered for 2022 Summer Day Camp. For questions, please email the team at childcare@athensmcminnymca.org.
